2. Traffic Data Collected.
We automatically track and collect the following categories of information when you visit our Site: (1) types of web browsers used to access the Site; (2) referring source which may have sent you to the Site; (3) types of computers accessing the Site; (4) IP addresses; (5) domain servers; and (6) other information associated with the interaction of your browser and the Site (collectively “Traffic Data”).
3. How Fast Orthodontics Uses Your Information.
Use of the Services may require Fast Orthodontics to use your Personal Information to allow your Providers to refer you to, and make appointments with, other Service Providers on your behalf. Your contact data may be used to send you information about Fast Orthodontics or our Services or products, to contact you when necessary, including but not limited to, reminding you of upcoming or follow-up appointments, and in conjunction with your use of certain interactive tools. We may use your insurance data, demographic data, personal needs data or Traffic Data, to customize and tailor your experience through the Services, in other communications and in emails, displaying content according to your preferences and that which we think you might be interested in. We may use your Personal Information to recommend certain resources or Service Providers. We may also use your non-identifying Personal Information to perform (or authorize third parties to perform) statistical research on individual or aggregate service industry trends. Such research would only use your Personal Information in an anonymous manner that cannot be tied directly back to you.
4. Personal Information Collected.
We may require you to provide us with certain information that personally identifies you (“Personal Information”) in order for you to access some areas of the Site and/or certain Services when it is important for us to know more about you so that we can best meet your needs. Any information you provide in any communications you send to us for example, by e-mail or letter, may be collected as Personal Information. Personal Information includes the following types of information: (1) demographic data (such as your gender, your date of birth and your postcode or zip); (2) contact data (such as your e-mail address and phone); and (3) service needs data (such as course of orthodontic treatment); and (4) your reason for visiting Service Providers, your date of visiting Service Providers, your service use history, and other information you choose to share with us.
5. Interactive and Installed Tools.
Some of the free Interactive Tools on our Site ask you for certain Personal Information. In order for you to use certain Services available through the Site you will need to download and install software on your computer (“Installed Tools”). These Installed Tools may collect and transmit information from your computer system relating to use of the Installed Tools and for the purpose of providing you the relevant Services, for example, by informing Fast Orthodontics when a User is logged on and available to receive updates, or a message informing you of an upcoming appointment, or a change in appointment time (“Notification”). We also collect other information (some of which may be Personal Information) that you voluntarily provide to us when you choose to use some of the Site Services. Interactive tools on the Site and Services (“Interactive Tools”), such as searching for Service Providers, searching for available appointments with Service Providers, and completing forms on the Site (“Site Forms”) prior to Service Provider appointments. Information you provide voluntarily in free-form text boxes on the Site and through responses to surveys and feedback forms, questionnaires and the like are also collected.
6. Web Beacons.
In limited circumstances, we also may use small graphic image files embedded in an email or web page that provide a presence on the email or web page and send back to its originating server information from the User’s browser (“Web Beacons”) to collect anonymous, non-personal information about your use of our Services, the sites of selected sponsors and advertisers, and your use of emails, special promotions or other communications we send to you. Information collected by Web Beacons enable us to monitor how people are opening our emails, and for what purposes, or using the Services and selected sponsors’ and advertisers’ sites.
8. Website Analytics.
We often use third party website analytics services in connection with the Services, for example, to record mouse movements, mouse clicks, scrolling activity, as well as any text that you type into the Site. These website analytics services do not collect Personal Information that you do not voluntarily enter into the Site. These services do not track your browsing habits across websites which do not use their services. The information collected from these services is used by us to find usability problems so we may to make the Services more user friendly. The recordings only record anonymous user information and they will never identify you or your account.
Storage of all Traffic Data and reviewing of postings are done indefinitely; even after “deletion”, we may archive such information. All of your Personal Information is stored until you request that it be modified or deleted, and in such cases we may still retain certain Personal Information for the reasons described in Section 17 below.

The Services are not aimed at children, and Fast Orthodontics does not knowingly collect any information from individuals under the age of 13. COPPA severely restricts what information can be collected from children under the age of 13. For this reason, children under the age of 13 in the United States are prohibited from using the Services. If we become aware that we have received any information from an individual under the age of 13, we will use that information only to respond directly to that child (or a parent or legal guardian) to inform him or her that he or she cannot use the Services and subsequently we will delete that information from the servers we have control of.
12. Information Sharing.
o For us to customize your advertising interactions, we are likely to share Personal Information with marketing stakeholders and other third parties only on an aggregate (i.e., anonymous) basis;
o We may share your contact data with some of our stakeholders to enable them to send you Fast Orthodontics promotional materials. These stakeholders will only use your contact data to send you Fast Orthodontics promotional materials;
o You may choose to authorize Fast Orthodontics to share your Personal Information with selected Providers. For example, if you elect to complete a Service Address Form using the Services in advance of an appointment, we may share your Service Address Form with our Service Providers if you agree that we may do so;
o We may share your contact data, Traffic Data, demographic data, and Service Address data with Service Providers you choose to schedule through the Services;
o We may share your anonymous Personal Information with third parties to enable them to run statistical research on individual or aggregate service trends;
o We may share certain marketing data including, but not limited to, anonymous Personal Information and non-Personal Information, with our business partners to help determine the effectiveness of certain advertising campaigns;
o Fast Orthodontics does not rent, share, sell, trade or barter your Personal Information with any third parties for their promotional purposes;

You can choose not to provide us with any identifying Personal Information. However, in such an event, you will only be able to access a limited number of our Services and you will not be able to access and use certain key features of the Services that require your Personal Information.

You should think carefully before disclosing any personally identifiable information in any public forum because any information that you will reveal in a review posting or other online discussion or forum is intentionally open to the public and is not in any way private. What you write is highly likely to be collected and/or seen by third parties and may be used by others in ways we are unable to predict or control.

As a registered User of the Services, you can modify some of the Personal Information you have included in your profile or change your Username by logging in and accessing your account. If, at any time you wish to close your account, you should email: firstname.lastname@example.org. Upon your request, Fast Orthodontics will use commercially reasonable efforts to delete your account and the Personal Information in your profile; however, it may be impossible to remove your account without some residual information being retained by Fast Orthodontics. Fast Orthodontics has no obligation to delete any data or information you provide on a Service Address Form following termination of your account or the account of any Service Provider to whom your Service Address Form was submitted through the Services.
19. Stolen or Lost Information.
If your contact data is lost, stolen, or used without permission, you must promptly notify us. In such an event, we will remove that contact data from your account and update our records accordingly.
